Is your child in Year 7 or Year 10?

NSW Health team are visiting the school on March 28 2023 for Year 7 and May 30 2023 for Year 10. To make the process easier you can provide your consent for their routine school vaccinations online.

In Year 7, students are offered vaccines for diphtheria-tetanus-pertussis (dTpa) and human papillomavirus (HPV). In Year 10, students are offered the meningococcal ACWY vaccine.

To provide online consent follow these steps:

• press search
  
• click on the first link
  
• scroll to ‘Consent for school vaccination’
  
• Click on ‘online consent portal’
  
• Follow the prompts to log into Service NSW, verify the parents details and provide consent for the child
  

Details you will need to complete the consent process

• Your Service NSW log-in details
• Medicare card details for you and your child If you or your child do not have a Medicare card, consent can still be provided by requesting a paper-based consent form directly from our school.
